HRTMOMMY • A FREE GIFT TO KEEP

The Before-You-Book Kit.

A few good questions. A place for the real numbers. A clearer conversation about your next step.

Use these three worksheets on your phone, print them, or save a PDF with your browser. You do not need to share your health information with us. The kit is available here even if you choose not to receive the email series.

Choose “Save as PDF” in your print menu to keep a copy. Any notes you add to a saved or printed copy stay with you.

01

BRING YOUR QUESTIONS

Twelve things you can ask.

Choose the questions that matter to you. You can read them straight from your phone, or circle them on a printed copy. This worksheet prepares a conversation; it does not choose a medicine or dose.

The care

  • What are the options for the concerns I want help with, including options without medication?
  • Which options fit my medical history, and which should I avoid?
  • What would make an in-person visit or a referral the better next step?

The medicine

  • What exact medicine, strength and form are you proposing, and what is it meant to help with?
  • Is this particular product FDA-approved or compounded? If compounded, why is it being proposed for me?
  • What are the important risks, side effects and interactions for me, and what should I do if they occur?

The whole cost

  • What will I pay before approval, after approval and at the next renewal?
  • Are visits, medication, lab work, shipping and follow-up included or billed separately?
  • When can I cancel, and which charges become nonrefundable before anything ships?

The follow-up

  • How do I contact the prescribing clinician between visits?
  • When will we review the plan, and what should prompt me to get help sooner?
  • If the medicine is unavailable or unsuitable, what happens to my plan and my payment?

WRITE DOWN THE WHOLE PRICE

The true-cost worksheet.

A monthly price may leave questions unanswered. Ask for a written breakdown, then compare the same amount of care over the same length of time. Leave an item marked “not confirmed” until you have the answer.

COMPARE LIKE WITH LIKE

Use the same questions for both providers. A longer list of products or a lower advertised price does not tell you which care is appropriate for you.
