The appointment planner

The question planner runs in your browser. It uses predefined questions and does not ask for health information. Selections are held in page memory, are not sent to a server and are cleared on reload. Printing uses your browser’s print function; any printed or saved copy is under your control.

Site operation

We have not added advertising pixels or third-party analytics scripts. Our hosting and infrastructure providers may process ordinary connection information such as IP address, requested URL and browser information to deliver and secure the site. This does not mean browsing is anonymous to every network or service involved.

Other websites

Source links and provider links leave this publication. Featured provider links include campaign tags identifying the referring site and article. Those sites have their own privacy rules, and any information you submit there is handled by them. Our site does not receive medical intake answers from those forms.
